Procedure for obtaining Copies:

1. Appear in person at the Health Department
2. Be the person referred to in the birth certificate OR 
   Be the parent or legal guardian of the person referred to in the birth certificate
3. Have a birth date after 1942 Earlier birth records not yet available in database
4. Have Photo Identification
5. Obtain the form requesting a duplicate certificate.
6. Pay $30.00 Fee

After 4 Weeks of the Date of Death
