Birth certificates can be obtained Monday – Friday from 8am-4pm at 125 S. Lynchburg Street location on the 3rd floor.

You may also fax in your application to 410-778-6119 or email [email protected] along with a copy of your photo ID and form of payment.

Payments can be made by Credit Card and we will call you to obtain that information, or you can mail a check with your application to the address on the form.  Please call 410-778-1350 if you have other questions or concerns.

1. Appear in-person at the Health Department
2. You must be the person referred to on the birth certificate OR be the parent or legal guardian of the person referred to on the birth certificate.
3. Have a birth date after 1942 (earlier birth records are not yet available in the database).
4. Have Photo Identification
5. Obtain the form Requesting a Duplicate Certificate.
